All teams must register 6:00PM-10:00PM on Tuesday night, November 25th at Our House Restaurant in Farmingdale, NJ - http://www.ourhouserestaurant.net/. Anyone on your team can register. It need not be a team offical. If there is an emergency situation whereby you cannot attend registration, please contact Beth Huguenin at 732-604-8801.
Each team must bring to registration a copy of their official roster, player passes, medical release forms and permission to travel for out of state teams (please see reminders below). Birth Certificates will not be checked at registration but teams should have them in their possession in case of challenges from opponents.
In addition to these documents, each team will be required to fill out an emergency contact sheet and a Tournament Roster Card. These forms can be found below to speed up your registration process.

A few reminders:
Travel forms- Teams playing on their US Club Soccer roster do not require travel forms.
Teams playing on a US Youth Soccer Roster must present at registration their state approved travel forms.
Roster limit- U8, U9, & U10(8v8) have a limit of 14 players including guests. U12, U13, U14, U15, & U16 (11v11)have a roster limit of 18 players including guests. U17 have a roster limit of 22 players including guests. Please note that prior to each game only 18 are to be chosen and activated on your tournament roster and presetned to the field marshal.
Please note: US Club Soccer rosters may only bring the indicated number of players for their age group. For example, a USCS roster might have 26 players on it, but prior to Friday night registration you must select which 18 are eligible to travel and participate in this tournament. You may not rotate from the 26 during the tournament.
Guest players - Each team may utilize up to 3 guest players so long as you do not exceed the allowable roster limit when adding these players. Each guest player must have an original, current seasonal year, valid player pass. Each guest must have a medical release form, which need not be notarized. There is no required guest player form or a pre-approval of guest players. You need only bring the information to registration and have them written on your tournament roster copy.
Very important- Please note that player passes must be consistent with the roster for which they are to play under. A US Club Soccer roster may only have players who have a US Club Soccer player pass. A US Youth Soccer roster may only have players that have a US Youth Soccer player pass. The sanctions do not allow for crossing of the registration materials.
